In the case of any clinical suspicion, magnetic resonance imaging must be performed for confirmation of the diagnosis and an evaluation of the severity. We report the case of a fat embolism syndrome with a cerebral manifestation only due to a closed fracture of the mandibular bone, a neurological deterioration after a free interval was marked with a normal initial cerebral CT. Fat embolism syndrome is a serious complication of long bone fractures, characterized by respiratory, neurological and mucocutaneous signs, often with respiratory syndrome in the first place, however isolated severe brain syndrome remains exceptional.
